I/We __________________ (adopter’s name) agree that effective ____________________ (adoption date) Great Lakes Labrador Rescue, Inc. (GLLR) in consideration of the sum of $_250.00______, in receipt of which GLLR releases full responsibility and ownership for ________________________ (dog’s name) to the undersigned adopter. Adoption donations are considered non-refundable after thirty (30) days from the above adoption date or if terms of this agreement are not met. The undersigned adopter does hereby release and covenant to hold harmless GLLR, its directors, officers, and members from any claims, damages, costs or actions incurred as a result of this adoption or caused by the actions of the dog transferred herein. The adopter further agrees to abide by the terms of adoption listed below: Non-Breeding: It is understood that if not already done, this dog will be neutered or spayed as soon as possible in accordance with a veterinarian's recommendation considering the dog's age and health. The adopter will provide GLLR with a copy of the vet's record showing this surgery has been performed. Transfer of Ownership: This dog will not be transferred to any other person by the adopter. Health: The dog will be placed on an inoculation program as prescribed by a licensed veterinarian to assure and maintain the health of the dog. The dog will receive a rabies vaccination and be licensed in accordance with state and local ordinances. The dog will be tested for heartworm and a heartworm preventative will be administered as prescribed by a veterinarian. The above program will be maintained on a yearly basis. Control: The dog will not be allowed to run loose off the adopter's property (except if involved in bona fide sporting events). THE DOG WILL BE UNDER VOICE CONTROL OR ON A LEAD AT ALL TIMES. The dog will be enrolled in obedience classes within 15 days of adoption and must provide proof of enrollment. (Adopter must initial) Care: The adopter accepts full responsibility for maintaining a reasonable level of care for this dog including but not limited to food, water, grooming, adequate sanitation, exercise, attention, safety, shelter, and prompt veterinarian diagnosis and treatment of medical conditions. Acceptance: GLLR has provided the adopter, to the fullest extent possible, information about the health, history and background of the dog. The adopter accepts responsibility for the dog based on the information available and provided at the time of this Agreement. Reservation of Rights: GLLR reserves the right to an ongoing review of this adoption and to follow up on any complaints or reports to protect the welfare of this dog. If the terms of this Agreement are not satisfactorily upheld by the adopter, and/or if any misrepresentations have been made to GLLR in order to obtain the dog, GLLR reserves the right to void the Agreement and demand the immediate return of the dog to GLLR by the adopter. |
