I / we, ____________________________________________________________________________________________ agree to act as temporary foster home for Great Lakes Labrador Rescue, Inc. (GLLR). I understand the following conditions will apply to fostering any GLLR dog while the dog is in my care or in my home. |

GLLR will retain legal ownership of the dog until the date of adoption. GLLR will continue to evaluate the dog while in the foster home; if GLLR determines a change in fostering is necessary, the dog will be surrendered to GLLR immediately on request. The fostering period can last from a few days to a few months, depending on the adoptability and health of the dog. A GLLR identification tag will be provided and must remain on the dog’s collar at all times. The foster family accepts responsibility for any and all events that occur in connection with the fostering of a GLLR dog. The foster family may be asked to help transport the dog to veterinary appointments or to meet applicants or adopters. Alternative arrangements will be made if the foster family is unable to transport the dog. To ensure the emotional well being of the Lab, he/she will be housed indoors as a companion unless otherwise specified herein. The dog will not be left unattended on a chain, rope or tie-out device. The dog will not be allowed to run loose and will be kept on a leash at all times when outside a fenced yard. Should the dog get loose, GLLR will be contacted immediately.

The foster family is responsible for expenses occurred in the normal daily care of the dog. This includes dog food, treats, toys, leashes, any damage to the property while in foster care, any long distance phone calls, if necessary to contact GLLR or potential adopters, and costs or expenses for transporting the dog. Regular veterinary care will be reimbursed by GLLR. All such expenses (except for emergency situations) must be approved in advance by GLLR. Regular veterinary care includes vaccinations, spay/neuter, heartworm testing and treatment, monthly heartworm preventative, prescription medicines or other veterinarian-recommended care. All attempts to get discounted veterinary care should be made; consult with a GLLR volunteer for possible clinics in your area known to discount for rescue groups. Receipts for approved expenses must be attached to a completed Request for Reimbursement Form and be Submitted to Great Lakes Labrador Rescue, 105 E. Vallette PMB# 1175 Elmhurst, IL 60126. Unapproved expenses will not be reimbursed.

The dog will be adopted to its permanent home under the supervision of GLLR to a family pre-approved by GLLR. The privacy of adopters, applicants, GLLR representatives and volunteers will be respected at all times. All donations connected with GLLR or the adoption of a dog will be sent directly to Great Lakes Labrador Rescue 105 E. Vallette PMB# 1175 Elmhurst, IL 60126 As a GLLR foster, I agree to release, indemnify, and hold harmless Great Lakes Labrador Rescue, Inc. of any and all claims and damages known or unknown resulting from or in connection with my fostering of any dog.
