Welcome to the on-line adoption form. We encourage you to review the adoption requirements and procedures so you are familiar with all the steps involved. Please complete the form below and note that all fields in yellow are required. Incomplete forms will not be processed. After submitting the form, please wait 10 days to hear from a GLLR volunteer. If you have not heard from someone within 10 days, please contact us to check on your application status. It is entirely possible that we did not receive your form for whatever reason. Remember, we are all volunteers, with no paid staff, so please be patient and wait the ten days. Thanks!

If you do not currently have a fence, you will be required to have one installed prior to a dog being placed in your home.

Would you consider a Lab mix? In most cases, this may simply mean the dog is slightly smaller than a purebred, or has a slight color variation not allowed by purebred standards. The dog would still have the general characteristic & temperament of a Lab.

Great Lakes Lab Rescue is an all volunteer organization that can only place Labs into homes that have passed a home-visit. Please understand that we may not have a volunteer in your area.

Incomplete forms will not be processed!

Completion of this form does NOT guarantee that you will get one of our Labs.

Great Lakes Lab Rescue reserves the right to refuse or deny any application.

If any misrepresentations have been made, Great Lakes Lab Rescue will demand the immediate return of any adopted Lab.

PLEASE NOTE: You must send in your adoption application and pass the home visit before we can discuss any questions you have about specific dogs. We will not return phone messages nor answer e-mails about specific available labs on the web site, unless you have already been approved to adopt.
